POLICY INQUIRIES

CHANGE MODE OF PREMIUM PAYMENT
Clients can change the mode of their payment by doing the following:
1. Download and fill-up the Amendment form (should be dated, signed and witnessed)
2. Submit a signed explanation letter together with the amendment form to any Sales Office nearest you or send it via e-mail to mypolicy@philippineprudential.com
POLICY LOAN
Clients may obtain a loan for an amount not exceeding the Cash Value on the sole security of their Policy and its proper assignment.
To file a policy loan:
1. Visit in any of the Philippine Prudential Sales Office (Personal appearance of Policy Owner is required)
2. Submit (2) valid identification and signed letter and
3. Fill-up Policy Loan Application Form
CHANGE OF POLICY OWNER NAME, STATUS
Clients can update / change their names and status by submitting an accomplished Amendment form (should be dated, signed and witnessed) to the nearest Philippine Prudential Sales Office together with following documents:
1. Proof of Age – (Copy of Birth certificate, passport)
2. Photocopy of Marriage Contract. (if change is due to marriage)
3. Photocopy of any valid identification.
CHANGE OF BENEFICIARY DESIGNATION
If the Beneficiaries are designated as “Revocable” the Policy owner may delete any Beneficiary or designate new Beneficiaries and exercise any and all other rights and privileges under the Policy while in force. To change a revocable beneficiary, the policy owner must submit the following to any of the Philippine Prudential sales offices:
1. Signed request letter
2. Birth Certificate
3. Accomplished Amendment form (should be dated, signed and witnessed)
If the Beneficiary is designated as “Irrevocable”, notwithstanding any contrary provisions the consent of all such irrevocable Beneficiaries is required before the Policy owner make any changes to his Policy. To change an irrevocable beneficiary, the policy owner must submit the following to any Philippine Prudential sales office:
1. Signed request letter and consent letter of current beneficiary
2. Original and certified true copy of legal documents to support changes such as Birth Certificate, Marriage Certificate or adoption papers
3. Accomplished Amendment form (should be dated, signed and witnessed)
